
As envisioned by the Holden Clarke, our founder, a comprehensive system of services is necessary to help clients transitioning in our communities reach sobriety and independence. This system and philosophy, called a called “Continuum of Care”, strives to fulfill those requirements with five fundamental components: Chemical Dependency Treatment, Relapse Prevention, Residential Living, and Transition into sober living, and Aftercare.
The fundamental components of a Continuum of Care System are:
- Outreach and Assessment, which identifies a client’s or a families’ needs in order to make appropriate connections to facilities and services, and treatment guidelines.
- Immediate shelter and treatment for homeless clients in the throw of addiction – an alternative to the streets.
- Residential Living with primary treatment and relapse prevention modality
- Transitional Living with appropriate services. These services may include job training/placement, and independent living skills.
- Continued lifetime support for maintaining sobriety and assistance with handle a new life.
